Mifepristone

Mifepristone, also known as RU-486, is a medication typically used with misoprostol to bring about an abortion. This combination is more than 95% effective during the first 50 days of pregnancy. It is also effective in the second trimester of pregnancy. Two weeks after use effectiveness should be verified. It is taken by mouth.

Common side effects include abdominal pain, feeling tired, and vaginal bleeding. Serious side effects may include heavy vaginal bleeding, bacterial infection, and a malformed baby if the pregnancy does not end. If used, appropriate follow up care needs to be available. Mifepristone works by blocking the effects of progesterone and causing contractions of the uterus.

Medical uses

Abortion

Mifepristone followed by a prostaglandin analog (misoprostol or gemeprost) is used for medical abortion.

A 2011 evidence-based clinical guideline by the Royal College of Obstetricians and Gynaecologists (RCOG) says medical abortion using mifepristone followed by the prostaglandin analog misoprostol is effective and appropriate at any gestational age.[13]

A 2012 technical and policy guidance book and a 2014 clinical practice handbook by the World Health Organization (WHO) recommend mifepristone followed by the prostaglandin analog misoprostol for first and second trimester medical abortions.

2013 and 2014 practice bulletins by the American College of Obstetricians and Gynecologists (ACOG) recommend mifepristone followed by the prostaglandin analog misoprostol for first and second trimester medical abortions.

Mifepristone alone results in abortion within two weeks of 54% to 92% of pregnancies.

Cushing's syndrome

Mifepristone is used for the medical treatment of high blood sugar (hyperglycemia) caused by high cortisol levels in the blood (hypercortisolism) in adults with endogenous Cushing’s syndrome who have type 2 diabetes mellitus or glucose intolerance and have failed surgery or cannot have surgery.

Vitamin D refers to a group of fat-soluble secosteroids responsible for increasing intestinal absorption of calcium, iron, magnesium, phosphate, and zinc. Retapamulin

Retapamulin is a topical antibiotic developed by GlaxoSmithKline. It is the first drug in the new class of pleuromutilin antibiotics to be approved for human use. It is marketed as an ointment under the brand names Altabax and Altargo.

Aliskiren

Aliskiren (trade names Tekturna and Rasilez) is the first in a class of drugs called direct renin inhibitors. Its is used for essential (primary) hypertension. While used for high blood pressure, other better studied medications are typically recommended due to concerns of higher side effects and less evidence of benefit.

Angiotensin II has both direct and indirect effects on blood pressure. It directly causes arterial smooth muscle to contract, leading to vasoconstriction and increased blood pressure. Angiotensin II also stimulates the production of aldosterone from the adrenal cortex, which causes the tubules of the kidneys to increase reabsorption of sodium, with water following, thereby increasing plasma volume, and thus blood pressure. Aliskiren binds to the S3bp binding site of renin, essential for its activity. Binding to this pocket prevents the conversion of angiotensinogen to angiotensin I. Aliskiren is also available as combination therapy with hydrochlorothiazide.

Metronidazole

Metronidazole (MNZ), marketed under the brand name Flagyl among others, is an antibiotic and antiprotozoal medication. It is used either alone or with other antibiotics to treat pelvic inflammatory disease, endocarditis, and bacterial vaginosis. It is effective for dracunculiasis, giardiasis, trichomoniasis, and amebiasis. It is the drug of choice for a first episode of mild-to-moderate Clostridium difficile colitis. Metronidazole is available by mouth, as a cream, and intravenously.

Common side effects include nausea, a metallic taste, loss of appetite, and headaches. Occasionally seizures or allergies to the medication may occur. Metronidazole should not be used in early pregnancy but appears to be safe later in pregnancy. It should not be used when breastfeeding.
